REVIEW THE FOLLOWING IMPORTANT INFORMATION
(A)CHECK TRANSPORT CANADA REQUIREMENTS
(B)ECG:
• If required, download and PRINT the ECG requisition and take the requisition to DYNACARE LAB(www.dynacare.ca) at least 1(one) week prior to your appointment to complete your ECG; PLEASE DO NOT USE LIFELABS for your ECG;
• ECG results will be sent automatically a few days later to the office if you use this printed requisition;
• If your are RENEWING an existing medical, your ECG must be completed and reviewed PRIOR to your appointment, otherwise your medical document will not be stamped.
(C)AUDIOGRAM:
•Available in-office for $75+HST.
(D) REPORTS:
Please send in advance any other useful/relevant/required documents such as test results, medical/specialist reports that may be required for specific illnesses such as:
- recent surgeries, injuries, etc
- depression,
- ADHD/ADD,
- diabetes (see resource page for required documents),
- sleep apnea: submit CPAP compliance report,
- laser eye surgery/refractive eye surgery,
- medication list
Any delayed submission of additional documents uploaded to Transport Canada after your appointment will incur an admin fee of $125+HST.
(D)FAA MEDXPRESS
• Complete your FAA MedXpress information and email your confirmation code to office@pilotMD.ca
• Submit existing Special Issuance (SI) or SODA documents if required.

YOUR APPOINTMENT
• APPOINTMENT TIME: There will likely be patients before and after your appointment slot, so arriving on time (not early) is best;
• DOCUMENT: For renewals, bring your Transport Canada ADB(“blue book”) or medical certificate to be stamped;
• GLASSES/CONTACT LENSES: If required, wear your glasses and bring your contact lenses and lens kit;
• RECEIPT: Emailed to you after your appointment.
